Chronic care

Diabetes, high blood pressure, thyroid, asthma and COPD, managed over time.

Long-term conditions go better with the same doctor at every visit. We see most patients with diabetes or high blood pressure every three to six months, check the labs that matter for your condition, and adjust medication based on the numbers.

Between visits, you can send readings and refill requests through the patient portal, and a nurse replies within one business day. Patients on Medicare with two or more conditions can join our care management program, which Medicare covers with a small monthly copay.

Yes. Sign a records release at your first visit and we request everything from your previous office.
